Family Health Teams are healthcare organizations that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide health care for their community. The clinic provides a variety of primary care programs and services that are focused on health prevention and educating and helping people manage their chronic conditions. It also provides a series of workshops run by social workers, dietitians, nurses, and Diabetes Education Program.
